How to fill out your change request form:

1. Leave the Scope or Timeline checkboxes unchecked in step 1.

2. In step 2, explain how your scope or timeline will be impacted by your increased personnel costs. Explain why your costs have increased and why the increases were unexpected.

3. In step 3, describe how (or if) your work will continue if the change request is denied. Be specific about which project deliverables will be impacted.

4. Next, if appropriate, list any NEW cost share (cash or in-kind) that your project will attract due to your project's increased budget. Include names of cost share partners.

5. In the budget section, click the Add button next to the Within-Year line.

6. Enter the dollar values of the Requested Adjustment in the rows of the appropriate Fiscal Year(s). Tab or move the cursor to the next field and the adjusted budgets will display in the Summary section.

7. In the Author and Contact section, your information is loaded by default. If the Primary Contact for your request is someone other than you, select the contact from the dropdown menu.

8. Click the "Save" button.

9. When you're satisfied with your request, click the Submit To PM button to send this to your BPA Project Manager who will be notified via email.

You are a proponent who encountered design and permitting delays that prevented the installation of some structures during the field season. You need to reschedule $118,000 from this FY to the next FY so all tasks will occur during a single field season.

How to fill out your change request form:

1. Most likely you will want to check the Timeline and checkbox in step 1.

2. In step 2, explain why the design and permitting was delayed and how the delay has impacted your project's work. Explain why it is important that all the work be completed within a single field season.

3. In step 3, describe how (or if) your work will continue if the change request is denied. Be specific about which project deliverables will be impacted.

4. Next, if appropriate, list any NEW cost share (cash or in-kind) that your project will attract due to rescheduling part of its budget. Include names of cost share partners.

5. In the budget section, click the Add button next to the Reschedule line.

6. Enter the dollar values of the Requested Adjustment in the rows of the appropriate fiscal year(s), and indicate which fiscal years should receive the rescheduled budgets. Tab or move the cursor to the next field and the adjusted budget will display in the Summary section.

7. In the Author and Contact section, your information is loaded by default. If the Primary Contact for your request is someone other than you, select the contact from the dropdown menu.

8. Click the Save button.

9. When you're satisfied with your request, click the Submit To PM button to send this to your BPA Project Manager who will be notified via email.
